Before filling out a nomination form, please review our qualifying criteria below. If your nomination meets our criteria, it will then move on to our selection process where a member of our team will reach out to you to conduct the selection interview. The selection committee will then make a final determination as to whether the project is approved. Once approved, the project moves into our planning and execution phase.
Qualification Criteria
- A legal resident or Citizen of the United States of America
- Not convicted of a felon
- Able to obtain approval from a doctor, if necessary
- Unable to fulfill the project on your own
- You are someone in need (including persons with chronic illness, disability or elderly)
